University of Naples Federico II — Medicine and Surgery (Napoli Federico II University)
The University of Naples Federico II offers the English-taught Medicine and Surgery program in Naples, under the Department of Translational Medical Sciences / School of Medicine and Surgery. The expression “Medicine and Surgery (ENG)” is a descriptive adjective used in orientation materials; it is not the official title of the program — the official registration name is simply “Medicine and Surgery.” The program is in the LM-41 class, is a six-year (12-semester) single-cycle Master's Degree, and has a total of 360 CFU/ECTS.
When evaluating the program, not only the IMAT cut-off should be considered; the 25 EU/45 Non-EU quota, the ADISURC Campania regional tax system restructured for 2026/2027 (167 €/year for Non-EU), the Universitaly + Segrepass registration process directly instead of MoveIN/Universitaly, the ADISURC scholarship, document eligibility, and the context of YKS should be considered together.

Non-EU students are exempt from the main Sapienza ISEE-based contribution (contributo onnicomprensivo, up to €2,384 for Italian students); they only pay the regional tax (€151, for families residing abroad) + €16 stamp duty. If the declaration of residence abroad is not made or is incomplete, the student falls into the "sprovvisto" (undocumented) category (€173 + €16 = €189).
Tuition Fee
€167 / year (Non-EU basic) — €189 if declaration is incomplete
English Certificate (Consulate Context)
Federico II does not list a separate English certificate as a mandatory university admission requirement for registration in the Medicine and Surgery track conducted in English; English proficiency is essentially considered proven through the IMAT. However, Italian embassies/consulates may require proof, such as IELTS/TOEFL, for visa assessment. Candidates should check with the Italian consulate in their country and, if possible, prepare a recognized certificate.
Visa Application Deadline

The Relationship Between ADISURC and Tuition Fee
The ADISURC application is a completely separate process from the Federico II tuition fee; admission is not guaranteed and it is generally initiated before the IMAT exam, before acceptance is confirmed.
Quota and Non-EU Cut-off Data

The EU and Non-EU quota/cut-off columns show the official ranking data published in the relevant years. Past cut-off values do not guarantee acceptance for future periods.
The quota for 2025/2026 has been verified through a direct review of the official admission notice (Prot. DR/2025/3685, approving D.R. 1128 of 13.3.2025). It is stated exactly in Article 1: "70 places... 25 places reserved for candidates who are EU citizens and non-EU citizens residing in Italy; 45 places reserved for non-EU candidates residing abroad." That is: 25 EU/Italy-resident + 45 Non-EU residing abroad = total 70. No separate quota has been reported for Marco Polo for this track.
These 25/45/70 figures reflect the 2025/2026 cycle, which is the most current official quota cycle. As of the date this page was prepared, the official individual rectorate decree for 2026/2027 has not been fully published; instead of leaving the quota blank, the 2025/2026 figures are shown as the most reliable existing reference (baseline), and it will be updated once the 2026/2027 decree is published.
The official threshold rule validated in the same band (Article 7): EU/Italy-resident candidates must score at least 20/90 points to be included in the ranking; those below this threshold are not included in the ranking. This 20-point threshold does not apply to non-EU candidates residing abroad — any score above zero is sufficient, but candidates who leave the exam paper completely blank (answering no questions) are excluded from the ranking. These threshold values should not be interpreted as a realistic passing score; they are only the legal minimum requirement to enter the ranking.
Rankings for Non-EU and EU/Italy residents operate through different mechanisms: For EU/Italy-resident candidates, the ranking is prepared by CINECA at the national level as a single list; for Non-EU candidates residing abroad, the ranking is prepared directly by the university (Federico II), separately only for its own quota of 45 places.
An official, publicly available, dynamic CINECA scrolling (moving ranking) data-based net final cut-off PDF could not be found during this research process. Therefore, this page does not use third-party cut-off sources and does not estimate ranking positions. The above cut-off score of 63.1 has been taken from the site's own current historical IMAT cut-off database (2025/2026, 45 Non-EU seats — matching the official quota in effect); it is not presented as a new, independent source.
Program and training structure
The Medicine and Surgery program is a six-year (12 semesters) single-cycle Master's Degree in the LM-41 class (P11-DH6) and comprises a total of 360 CFU/ECTS. The program is administratively carried out by the Department of Translational Medical Sciences (Dipartimento di Scienze Mediche Traslazionali) under the School of Medicine and Surgery (Scuola di Medicina e Chirurgia). Course and laboratory facilities are located in Naples' medical district, within the university hospital network (Azienda Ospedaliera Universitaria) and community health structures; there is no additional/auxiliary campus for this English track.
The curriculum is spread over 12 terms and includes 34 integrated courses. The program combines clinical and basic sciences at an early stage; in the first and second years, clinical cases are introduced to encourage translational medical thinking.
Attendance to lessons, clinical practices, and laboratory sessions is strictly mandatory; students must complete all attendance requirements to be able to take the final exams.
OFA (additional education obligation) has been defined as a general admission barrier in the research process, and no isolated local threshold has been found; individual performance profiles are subject to national regulations. Therefore, this page does not claim a specific OFA threshold; national admission rules and course regulations should be checked separately every year.
Clinical clerkships and medical internships begin in the third year and continue with increasing intensity until the sixth year. Students are required to complete a clinical internship of at least 18 months at an accredited medical institution under the supervision of an academic advisor; this internship is a prerequisite for the final graduation exam. According to Italian legislation (Decreto-Legge n. 18/2020), this exam also serves as the State Exam; that is, graduates gain the direct right to practice medicine in Italy.
For graduation, students are required to write and defend an original thesis under the supervision of an academic advisor. Experimental theses must be assigned at least 18 months before graduation, while review/bibliographic theses must be assigned at least 6 months before.
There is no official Italian B2 requirement for registration or visa procedures. However, practical communication with patients during clinical rotations starting from the third year makes practical knowledge of Italian at a conversational level necessary.
Tuition Fee
The basic annual fee for non-EU students residing abroad for 2026/2027 is €167 (€151 regional tax + €16 stamp duty); this is different from the figure of €156/year (€140 + €16) used in previous periods — ADISURC Campania has restructured the regional tax into a three-tier system.
If the family residence abroad is not correctly declared or no document is submitted at all ('sprovvisto'), the student automatically falls to the highest tier (€173 + €16 = €189/year). Therefore, it is important that the declaration of residence abroad is completed fully at the time of registration.
ISEE Parificato IS NOT required for calculating the Federico II tuition fee for non-EU students; the fee is automatically determined based directly on citizenship/residency status. ISEE Parificato is only mandatory for the ADISURC Campania regional scholarship application — these two processes are completely separate from each other.
These international fixed fees are not dependent on academic success and are not affected by domestic tuition fee exemptions. However, students who win the ADISURC scholarship receive a full refund of the regional tax they paid (€151 or €173).
Exact installment dates depend on the registration (immatricolazione) calendar; the first installment becomes due on October 31, 2026 (standard registration deadline).
2026/2027
From the 2026/2027 academic year, ADISURC Campania has changed the regional tax (tassa regionale per il diritto allo studio) to a three-tier system instead of the previous fixed rate of €140: Tier 1 (€130) for students with an ISEEU value of less than €25,500 or who are citizens of developing countries according to the MUR list; Tier 2 (€151) for students with an ISEEU value between €25,500-51,000 OR whose main family place of residence is abroad; Tier 3 (€173) for students with an ISEEU value exceeding €51,000 or who have not submitted any economic/residence documents at the time of payment ("sprovvisto di attestazione"). Non-EU residing abroad students are completely exempt from the main income-based contribution (contributo onnicomprensivo), which can reach up to €2,384 for Italian students; they only pay the regional tax and stamp fee. The total annual fee for a Non-EU student who correctly declares his/her family residence is €151 (regional tax) + €16 (stamp fee) = €167. If the declaration is not made or is incomplete, the student is automatically dropped to Tier 3 (€173 + €16 = €189).
Scholarship and dormitory applications
The main scholarship and student rights institution in the Campania region is ADISURC Campania (Azienda per il Diritto allo Studio Universitario della Regione Campania). The institution is responsible for student welfare services, including financial support, accommodation, and meal services.
Students who win the ADISURC scholarship are refunded in full the regional tax (151 € or 173 €) they paid during registration. Additionally, the scholarship program offers heavily subsidized or completely free accommodation in university dormitories — this is a significant advantage, as private studio apartments in Naples can range from 250 € to over 450 € per month depending on their proximity to the city center. The scholarship may also include free or subsidized meals in university cafeterias and communal dining halls, as well as cash support according to the student's financial situation/distance category (Fuori Sede).
Although international students do not need the ISEE Parificato for Federico II tuition fees, the ISEE Universitario Parificato (ISEEUP) is absolutely required for students who wish to apply for any of the ADISURC scholarships, accommodation, or meal entitlements. This document must be calculated by an authorized CAF (Centro di Assistenza Fiscale) officially partnered with ADISURC; the family's foreign income, household structure, and real estate assets must be translated, legalized, and submitted.
The ADISURC application window is extremely narrow and usually closes in mid-September (for the 2025/2026 cycle, it closed on September 18, 2025). The exact deadline for 2026/2027 has not been officially published as of the date this page was prepared; the past cycle is provided for reference only and does not guarantee anything for the next year. This means that students must complete the scholarship application and initial documents before taking the IMAT exam or before their admission is confirmed.
In order to receive the final scholarship allocation, students must verify and finalize their university registrations by the deadline set in winter (usually the end of February).
A scholarship is not an absolute right; the application is subject to ADISURC's approval and the call conditions of the relevant year.
Non-EU application roadmap
- 1
Universitaly pre-enrollment15 Haziran 2026
Non-EU candidates who require a visa must complete the pre-enrollment procedure through the national Universitaly portal before university registration. The deadline for the 2026/2027 academic year is June 15, 2026; after this date, changes to program choices will not be allowed. This is a critical early date that comes before the IMAT exam.
- 2
IMAT exam and ranking
Admission is entirely based on the national IMAT exam. The 2026 IMAT national exam date is September 29, 2026, according to the MUR calendar. The IMAT registration window and Federico II's local ranking, final registration, and document submission dates should also be checked from the updated official announcement.
- 3
University registration (immatricolazione) — Segrepass portal16 July – 31 October 2026 (first year registration)
First-year registration is a process that opens after the IMAT ranking, independent of Universitaly, and is completed online through the Segrepass portal (segrepass1.unina.it). For 2026/2027, the registration window is from July 16 to October 31, 2026. September 1, 2026, is only the opening date for administrative registration for the second and subsequent years — it is NOT the opening date for first-year registration; these two dates should not be confused.
- 4
Late registration penalties1 November 2026 – 15 January 2027: €50 · After 16 January 2027: €110
Records after October 31, 2026, are subject to an additional fee: a 50 € penalty will be applied for late registrations between November 1, 2026, and January 15, 2027; from January 16, 2027, this penalty rises to 110 €.
- 5
Visa application deadlineNovember 30, 2026
Non-EU candidates who require a visa must submit their final visa application at Italian embassies/consulates no later than November 30, 2026.
Diploma Compatibility and Required Documents
- Valid passport — must remain valid throughout the duration of study.
- High school diploma — should indicate the completion of at least 12 years of formal education.
- Academic transcript that can be requested when necessary.
- Official translations, documents that have undergone legalization or apostille procedures — are required for foreign documents to be recognized by Italian authorities.
- Declaration of Value (DoV) — an official document issued by the Italian embassy/consulate in the country where the diploma was obtained, showing the equivalence of a foreign diploma; or CIMEA Statement (Comparability and Verification) — can be requested online through the CIMEA portal as an alternative to DoV.
- Universitaly pre-enrollment receipt — it is automatically generated when online pre-enrollment is completed.
- Type D entry visa ("Studio-Immatricolazione Università") — is obtained by the student from the Italian embassy/consulate in their own country.
- Codice Fiscale (Italian tax identification number) — can be obtained through the International Welcome Desk or the local tax office (Agenzia delle Entrate).
- Permesso di Soggiorno (residence permit) application — Receipt showing that the application was made at the post office within 8 days of arrival in Italy.
- For candidates applying from Turkey, the high school diploma must grant access to higher education in Turkey. Therefore, YKS, diploma eligibility, DoV/CIMEA, and consular documents should be evaluated together.
Ranking and registration process

Important considerations
- The non-EU quota offers a large international pool for Turkish candidates with 45 people (2025/2026, officially confirmed); however, the program should still be considered highly competitive.
- Tuition fees are a fixed and low amount for Non-EU students (167 €/year); however, the declaration of residence abroad must be completed in full at the time of registration, otherwise it will increase to 189 €/year.
- Universitaly pre-enrollment (June 15, 2026) and the visa application deadline (November 30, 2026) are both critical and separate processes that should not be confused.
- The first-year registration opens on July 16, NOT on September 1; September 1 is only the administrative registration opening date for the second and subsequent years.
- The ADISURC scholarship application usually closes in mid-September, before the IMAT exam; the exact 2026/2027 date has not yet been officially published.
- The requirement of 12 years of formal education is a national prerequisite for access to higher education in Italy and should be evaluated together with the YKS context for Turkish candidates.
- The 2026 IMAT national exam date is September 29, 2026. The 2026/2027 full quota confirmation, IMAT registration window (August 26–September 9, 2026, 15:00 GMT+2), and Federico II local dates will be updated separately when the latest announcements are published. For general schedule logic, the IMAT Application Calendar page can also be consulted.
Assessment for Turkish students
Editorial evaluation
For candidates applying from Turkey, the issue is not only the IMAT score. Universities in Italy check whether the foreign high school diploma grants access to higher education in the country where it was issued; in the context of Turkey, this is related to the YKS process. Therefore, the YKS, diploma eligibility, DoV/CIMEA, and consular documents should be evaluated together.
The official quota for Federico II for 2025/2026 has been confirmed: 25 EU/Italy-resident + 45 Non-EU residing abroad (total 70). The 45-person Non-EU quota offers a relatively large international pool among English-taught medical programs in Italy and can be considered a strategic advantage for Turkish candidates; however, it is still a highly competitive process.
There is a significant financial advantage regarding tuition fees: Non-EU students residing abroad are completely exempt from the main income-based contribution for Italian students, which can reach up to €2,384, and only pay a fixed regional tax + stamp duty of €167/year. ISEE Parificato is not required for this fee. However, if it is not correctly declared at the time of enrollment that the family's place of residence is abroad, the student automatically falls into a higher bracket of €189/year — therefore, Turkish candidates should fill out the registration form carefully.
There are three dates that are critical in terms of timing: Universitaly pre-enrollment (June 15, 2026), ADISURC scholarship application (usually mid-September, the exact date for 2026/2027 has not yet been published), and the visa application deadline (November 30, 2026). The Universitaly and ADISURC deadlines occur before the IMAT exam; Turkish candidates must start these processes before knowing their IMAT results.
Turkish students who want to apply for the ADISURC scholarship must have their family income and asset documents converted to ISEEUP through an authorized CAF and translated/legalized, even if not required for the Federico II tuition fee; these two processes (Federico II fee and ADISURC scholarship) are completely independent of each other.
Regarding English proficiency, Federico II does not list a separate English certificate as a mandatory university admission requirement — proficiency is essentially considered proven through the IMAT. However, Italian embassies/consulates may request proof such as IELTS/TOEFL for visa evaluation; this entirely depends on the consulate's practice. It is recommended that Turkish candidates prepare a recognized English certificate as a precaution and check the current practice of the Italian consulate in their country.
The cut-off assessment on this page is based on the site's own current historical IMAT cut-off database (2025/2026: 63.1); this does not constitute a guaranteed or target score commitment for the coming year. Non-EU and EU/Italy-resident rankings operate through different mechanisms: Non-EU candidates compete only for Federico II's 45 local quota, while EU/Italy-resident candidates are subject to a nationally scaled CINECA ranking.
Candidates who want to compare English medical options in Southern Italy should also check the University of Bari Aldo Moro page; Bari's income-based fee system based on ISEE Parificato is different from the fixed low fee logic on this page, and the two systems should not be confused with each other.
Frequently asked questions
Is Federico II Medicine and Surgery in English?
Yes. The program is conducted entirely in English under the Department of Translational Medical Sciences / School of Medicine and Surgery in Naples. The phrase "Medicine and Surgery (ENG)" is a descriptive orientation addition; the official registration name is only "Medicine and Surgery."
What is the Non-EU quota for Federico II?
It has been confirmed by directly reviewing the official admission quota for 2025/2026 (D.R. 1128 of 13.3.2025): 25 EU/Italy-resident + 45 Non-EU residing abroad (total 70). This is the most recent official quota cycle; official figures for 2026/2027 have not yet been fully published.
Is there a Marco Polo quota?
For this track conducted in English, no separate Marco Polo quota has been specified in the official band or ministerial decree.
How much is the tuition fee for non-EU students at Federico II?
The basic annual fee for 2026/2027 is €167 (€151 regional tax + €16 stamp duty). Non-EU students residing abroad are exempt from the main contribution fee based on income, which can reach up to €2,384 for Italian students. If the declaration of residence abroad is missing or not made, the student automatically falls into a higher bracket of €189/year (€173 + €16).
Is the old figure of 156 €/year still valid?
No. ADISURC Campania has restructured the regional tax (previously a fixed €140) into a three-tier system (€130 / €151 / €173) starting from the 2026/2027 academic year. For students with families residing abroad, the current total fee, including the €16 stamp duty, is €167.
Does ISEE reduce the Federico II tuition fee?
No, this is not necessary. For non-EU students, the tuition fee is automatically fixed based on citizenship/residency status; there is no need to submit the ISEE Parificato. The ISEE Parificato is only required for the ADISURC Campania regional scholarship application.
When is the Universitaly pre-enrollment deadline?
The deadline for Non-EU candidates requiring a visa for the 2026/2027 term is June 15, 2026. No changes to preferences will be allowed after this date. This date replaces the former "mid-July" reference and comes before the IMAT exam.
When does university registration open?
First-year registration opens on July 16, 2026, and closes on October 31, 2026. The date of September 1 is only the administrative registration opening date for the second and subsequent years; it is not the opening date for first-year registration. These two dates should not be confused.
What is the deadline for the visa application?
Non-EU candidates who require a visa must submit their final visa application at Italian embassies/consulates no later than November 30, 2026.
Can Turkish students apply for the ADISURC Campania scholarship?
Yes. ADISURC provides rights such as scholarships, accommodation, and meal support; the regional tax paid in case of award is also fully reimbursed. For the application, an ISEEUP document showing the family's overseas income and assets, calculated through an authorized CAF, is required. The application window usually closes in mid-September; the exact date for 2026/2027 has not yet been officially published. Winning the scholarship is not guaranteed.
Is the YKS relevant for Turkey?
Yes. Federico II generally requires that the candidate's high school diploma allow access to higher education in their own country. In the context of Turkey, this is related to the YKS; Turkish candidates should consider YKS results, high school diploma eligibility, the DoV/CIMEA certificate, and the consulate process together. This page does not present a specific score type or document format as a strict requirement.
English certificate gerekiyor mu?
Federico II does not list a separate English certificate as a mandatory university admission requirement; English proficiency is essentially considered proven through the IMAT. However, the Italian embassy/consulates may request evidence such as IELTS/TOEFL for visa assessment; this depends on the consulate's practice. Candidates are advised to prepare a recognized certificate as a precautionary measure.
This guide is based on official sources from Federico II and ADISURC Campania. Quotas (25 EU / 45 Non-EU / 70 total), threshold rules, Universitaly pre-enrollment, and visa dates have been checked from relevant official sources. The updated institutional announcement of ADISURC Campania for the 2026/2027 tax brackets and the fees based on them should also be reviewed. Information will be updated once the full quota for 2026/2027, IMAT registration window, local Federico II dates, and ADISURC final application deadline are published.
